The hospital or clinic must be a Federal 340B Disproportionate Share Hospital or Clinic whose financial guidelines for indigent care are at least as strict as the following guidelines for patients. Non 340B hospitals whose guidelines for indigent care are the same or more strict may also apply. Patient Guidelines The patient must be a US resident, with no prescription coverage. The patient must also have an income at or below 200% of the Federal Poverty Level, and the physician/pharmacist must certify that the patient is not eligible or covered by any government or insurance programs.
